Instead of a calendar of isolated appointments, it maps your care across the year: which providers you need to see and how often, which follow-ups depend on which results, which referrals are still pending, and where the gaps are. If your rheumatologist wants labs before the next visit, and the labs need an order that hasn't been placed, the plan surfaces that before it becomes a cancelled appointment. This matters because complex care fails in the spaces between providers, not inside them. Each specialist manages their own piece competently and none of them owns the sequence. The patient does — usually from memory, usually while unwell. My Health Plan makes that sequence explicit so it can be managed rather than remembered.

A plan that only describes what should happen is just another list. This one connects to the rest of the app: when it's time to get in with a specialist, the booking request can go straight to the call team; when records are needed ahead of a visit, the request can go out automatically.
