A complaint goes to the Office for Civil Rights and must generally be filed within 180 days of when you knew about the problem. It needs to identify who you're complaining about, what happened, and when. The app walks the form section by section, keeps what you've already entered so you don't restart, and submits it for you. Most people never file, not because nothing happened but because the process is opaque and exhausting at exactly the moment they have least capacity for it. The right exists whether or not anyone tells you about it; the barrier is almost entirely procedural.
What you can file about
Improper use or disclosure of your health information, being denied access to your own records, having a correction request ignored, or being charged unreasonable fees for copies.
Complaints can also cover violations of civil rights or of conscience and religious freedom protections in a healthcare setting.
The 180-day clock
There's a deadline: generally 180 days from when you knew or should have known about the violation, though it can be extended for good cause.
That window is one of the main reasons complaints don't get filed. People spend months trying to resolve something directly with a provider and find the deadline has passed by the time they give up.
